Template for processing link elements
This template matches link elements. There are four
possibilities to consider: the link may have content or it may be
empty and the link may have an xlink:href
attribute or a linkend attribute:
Has Content
Is Empty
Has xlink:href
Content is a link to the specified URI.
The specified URI is used as the content. It is a link to the
specified URI.
Has linkend
Content is a link to the element identified.
Semantically equivalent to an xref to the element
identified.
Attempt to link to undefined ID:

Template for processing xref elements
This template matches xref elements. There are two
possibilities to consider: the xref may have an
xlink:href
attribute or a linkend attribute
For the moment, I don't know what to do about the href case.

Mode for inserting title markup
This mode is used to insert title markup. Any element processed
in this mode should generate its title.
Mode for inserting subtitle markup
This mode is used to insert subtitle markup. Any element processed
in this mode should generate its subtitle.
Mode for inserting label markup
This mode is used to insert label markup. Any element processed
in this mode should generate its label (number).
Mode for inserting page number markup
This mode is used to insert page number markup. Any element processed
in this mode should generate its page number.
Mode for inserting “direction” markup
This mode is used to insert “direction”. Any element processed
in this mode should generate its direction number. The direction is
calculated from a reference and a referent (above or below, for example).
Mode for inserting olink document name markup
This mode is used to insert olink document name markup.
Any element processed in this mode should generate its document name.
Mode for processing endterms
This mode is used to insert the markup associated with an
endterm .
Mode for removing IDs
This mode processes result (XHTML) markup, removing all ID
attributes.
Mode for inserting cross reference prefixes
This mode is used to insert prefixes before a cross reference.
Any element processed in this mode should generate the prefix appropriate
to cross references to that element.
